Colonoscopy/Out-Patient Assistance
Freedom Home Care’s Colonoscopy/Out-Patient Program is designed to assist any individual preparing for a Colonoscopy or out-patient procedure.This program is scheduled as a 24 hour visit in which a Freedom Home Care certified nursing assistant or caregiver will help you in the process of preparing for, attending and recovering from a Colonoscopy or out-patient procedure.
Freedom Home Care caregivers may assist you in the following plan of care…
- Arrive at your home the evening before the procedure with the intent of staying for a 24 hour period
- Assist in following the Colonoscopy or out-patient preparation kit–including picking up the kit as needed
- Assist with any bathroom needs throughout the preparation course
- Maintain NPO (no intake by mouth) status
- Facilitate transportation to and from the procedure
- Monitor recovery from the procedure with meal preparation and/or light housekeeping as needed
Day-Surgery/Physician Appointment Assistance
Freedom Home Care provides individual assistance for all out-patient procedures, day surgery and physician appointments. This specific plan of care and time frame is based on the needs of the patient, physician’s orders and the type of procedure, surgery or appointment one is preparing for. These procedures/appointments may include cataract surgery; ophthalmology; podiatry; dermatology; or orthopedic, neurological, plastic or reconstructive surgeries. If you are having a procedure not listed, Freedom Home Care can customize a plan of care for your specific needs including transportation to and from the appointment or procedure as needed.
Home Sweet Home Program
In order to meet the changing needs of our clients and their families Freedom Home Care implemented our Home Sweet Home Program. This program was designed to assist patients and their families transitioning from any environment back to their home. The Freedom home Care assistant or caregiver will
- We will meet you at the hospital, skilled nursing facility, assisted or independent living facility to begin the transition from the very beginning
- Provide the proper transportation to the clients home
- Make sure all discharge prescriptions are filled and picked up in a timely manner
- Pick-up and deliver appropriate dietary provisions
- Assist the client in getting settled safely in their home
- Complete any necessary light housekeeping, laundry or meal preparation
New Baby Program
Freedom Home Care’s New Baby Program is designed to assist new mothers and fathers in their own home with special needs or special circumstances with a new baby, or multiple new babies. Whether the mother is remaining in the hospital for a longer period, the baby has special needs such as jaundice care or both responsible parties are unable to provide full care for the newborn, we can help with the assistance and care needed by the new family to help them ease into the care of the new baby or babies.
